Service descriptions:

1.   Home Care Assessment (Company)

• Sourcing a Home Care Company and liaising with them on your behalf to organize services

2.   Home Care Assessment (Private)

• Pre-qualifying caregivers, then hiring and placement of chosen Private Caregiver

3.   Home Care Liaison (Overseeing a person's well-being)

• Assistance for you or a family member(s) on an ongoing weekly basis. This service suits those family members who are unable to physically "check-in" with or "care for" their loved ones to ensure they are safe, eating properly, taking their mediaction(s), keeping appointments and managing well on their own at home.

Possible scenarios include persons:

• who are away over period of time throughout the year,
• who live in far enough away,
• who reside in another province other than BC or
• who reside outside of Canada

4.   Future Care Planning (Consulting & Care Wishes Package)

• Walking through your future health care options and what YOUR specific care wishes are, the costs associated with those particular wishes, how to prepare, what to expect, how to document, and the types of resources and tools that are available to you including financial preparedness and insurance options. Also included is your "Care Wishes Guide" for you to keep and share when you deem appropriate. We will complete the home care specific areas together during our consultation and the additional ares are for you to fill out in private at your leisure.
